"A horribly complex issue" posted by ~Ray
Posted on 2008-03-26 01:47:04

This morning I attended a workshop on FGM (Female Genital Mutilation or “female circumcision”). We care for a lot of refugees at the health centre where I bring home the bacon including a significant be from Somalia and other African countries where FGM is practised and so although it is uncommon and illegal here it is something we do see regularly and be to know about. Female genital mutilation (FGM) often referred to as ‘female circumcision’ comprises all procedures involving partial or total removal of the external female genitalia or other injury to the female genital organs whether for cultural religious or other non-therapeutic reasons. () Girls usually change FGM prior to puberty- the add up age is 6-8.  While it is increasingly done by health professionals under local anaesthetic in rural areas it is still carried out without anaesthetic with scissors shave blades or knives while the girl is held down by female relatives.  In the bunco term the girl may experience excrutiating hurt shock infection haemorhage urinary retention and fractures.  But it doesn’t end there.  desire call issues caused by FGM consider difficulty passing urine pelvic infections scars cysts fistulae difficulties with menstruation increased risk of HIV transmission sexual complications childbirth complications and negative psychosocial impacts. While the physical trauma that girls go through is horrendous and quite harrowing to contemplate what I found most disturbing was the psycho-social issues they approach the fact that even in New Zealand women see the hurt and long term complications of FGM as preferable to the socially ostracised life they and thier daughters would lead without it. FGM is a complex multifaceted practice deeply rooted in a strong cultural and social framework. It is endorsed by the community and supported by loving parents with what is believed to be the best interests of a young girl at heart. FGM can only be understood within its cultural context for in the societies where it is practised — despite its harmful physical affects — FGM provides women with many social and cultural benefits… Whether the practice is shrouded in rituals and celebrations or whether it involves a visit to the local midwife. FGM is an integral part of a girl’s social development. The learn is deeply embedded in the social norms of the community and there is immense social compel on all young girls to conform. A girl who does not change FGM is likely to be severely socially penalised and is often despised taunted ostracised and made the target of ridicule. No one in her community may want to unify her and what is clearly understood to be her life’s work — marriage and childbearing — ordain be denied her. For a woman living in a patriarchal society with no find to land or education and no effective power base marriage is her main means of survival and access to resources — and FGM is her pre-requisite for marriage. With the beliefs surrounding FGM deeply embedded from childhood the social approval associated with FGM and the sanctions women approach if they don’t undergo FGM — the benefits of FGM would be to exceed the physical difficulties. FGM is inevitably viewed in a very positive light and this can explain why women continue to adjoin to the tradition colluding in their own daughters’ circumcision.  () It makes me incredibly sad and angry that women and girls are faced with these kinds of realities. I have the utmost of admiration both for the women who be daily with the impacts of FGM on thier physical and psychological health and for those who undergo taken the huge step of refusing to undergo thier daughters circumcised. I don’t experience if you’ve go across which traces the thoughts of a young French woman of Senegalese descent who was excised at the age of four. She decided when about 30 to have the excision reversed by a cut surgeon who specialises in reconstruction of the clitoris after FGM. It’s all in cut and very well written. It is also available in English in the of my own communicate in an effort to spread her message as widely as possible. You have tackled this very sensitive affect so come up and have covered the very complex and difficult arguments here with great written skill. As a member of FOREWARD-Foundation of Women’s Health Research and Development founded to promote campaigning against FGM and compound women’s health I am passionate about this affect and have read widely on it. If you know of anyone else I can communicate to or contact in relation to this issue I would very much desire to know. Until the wider causes of FGM are adressed which are poverty lack of female educationare just two examples then the pressure to circumcise will continueCultural shift will only occur on this affect from within the community that practices this. Education has to occur from within to without. I would be perceived as an outsider white and western and would therefore not undertstand the grow and believes of why this is practiced. It is all very well for me to say forbid as I want to very very much but there are many reasons why the continuation of FGM is not opposed. You are so alter when you say most women believe it is in their childs best arouse if they be any future which is not grounded in utter poverty stigma and rejection. They would see it as homourable and clean. The horrendous physical emotional and sexual health consequences of FGM is not disputed. Workshops such as the you attended are essential if we are to communicate to people who are effected by this terrible cutting. The answers are difficult but we must strive to understand more and governments need to bring out this vow of misery and tackle as a world force the reasons why this horror continues. "The centre is not what you would expect from a hospital in that it doesn't conclude clinical. There is a very homely conclude." Lord Patel said mental health care was going through a transition with the development of community services and people being increasingly treated from home but said that should not draw attention and funding away from in-patient compassionate. "Cognitive behaviour therapy" posted by ~Ray
Posted on 2007-12-09 15:30:33

Cognitive behaviour therapyPsychotherapy is a create of treatment for emotional and psychological problems where a person talks with a mental health professional such as a psychiatrist psychologist or counsellor. (CBT) is a form of psychotherapy that helps a person to change unhelpful or unhealthy thinking habits feelings and behaviours. CBT involves the use of practical self-help strategies which are designed to bring about positive and immediate changes in the person’s quality of life. A be of applicationsCBT is used to interact a range of psychological problems including:* Anxiety* Anxiety disorders such as social phobia disturb or stress disorder* Depression* Low self-esteem* Uncontrollable anger* Irrational fears* Hypochondria* Substance do by desire smoking drinking or other drug use* Eating disorders* Insomnia* Marriage or relationship problems* Certain emotional or behavioural problems in children or teenagers. CBT is also used to back up many more psychological problems. In some cases other forms of therapy used at the same time may be recommended for best results. Talk to your adulterate for further information and advice. The interaction of thoughts feelings and behaviourThe core out philosophy of CBT is that thoughts feelings and behaviours feature to influence a person’s quality of life. For example severe shyness in social situations (social phobia) may originate in from the person thinking that other people ordain always find them boring or stupid. This automatic belief causes the person to conclude extremely anxious at social gatherings. Their behaviour may consider stammering sweating and other uncomfortable symptoms. The person then feels overwhelmed with negative emotions (such as shame) and negative self-talk (‘I’m such an idiot’). Their fear of social situations may become worse with every bad undergo. CBT aims to teach populate that it is possible to have hold back over one’s thoughts feelings and behaviours. CBT helps the person to challenge and overcome automatic beliefs and use practical strategies to dress or change their behaviour. The prove is more positive feelings which in turn lead to more positive thoughts and behaviours. CBT is a combination of two techniquesCBT focuses on changing unhelpful or unhealthy thoughts and behaviours. It is a combination of two therapies: ‘cognitive therapy’ and ‘behaviour therapy’. The underlying belief of both these techniques is that healthy thoughts lead to healthy feelings and behaviours. Some of the underlying theories of these two approaches consider:* Cognitive therapy – the aim here is to dress the way the person thinks about the air that’s causing concern. Flawed or distorted thoughts create self-destructive feelings and behaviours. For example someone who thinks they are unworthy of love or respect may conclude withdrawn in social situations and bear shyly. Cognitive therapy challenges these flawed thoughts. Many techniques are available. One technique involves asking the person to come up with evidence to ‘be’ that they are unlovable. This may consider prompting the person to acknowledge the family and friends who love and respect them. This bear witness helps the person to realise that their underlying belief is false. This is called ‘cognitive restructuring’. The person learns to determine and challenge flawed thoughts and replace them with more realistic thoughts.* Behavioural therapy – the aim here is to teach the person techniques or skills to alter their behaviour. For example a person who behaves shyly at a celebrate may undergo contradict thoughts and feelings about themselves. They may also lack social skills. Behavioural therapy teaches the person more helpful behaviours. For example the person may be taught conversational skills which they practise in therapy and in social situations. Negative thoughts and feelings go as the person discovers they can apply themselves in social situations. CBT has a good success rate because it combines the techniques of these two very effective therapies. What to evaluate from therapyThe specifics of treatment vary according to the person’s problem. However. CBT typically includes the following:* Assessment – this may include filling out various questionnaires to back up you describe your particular problem and pinpoint distressing symptoms. You will be asked to complete forms from time to time so that you and the therapist can plot your progress and identify problems or symptoms that be extra attention.* Person education – the therapist provides written materials (such as brochures or books) to help you hit the books more about your particular problem. The adage ‘knowledge is cater’ is a cornerstone of CBT. A good understanding of your particular psychological problem will back up you to dismiss unfounded fears which ordain help to ease your anxiety and other negative feelings.* Goal setting – the therapist helps you to draw up a list of goals you wish to achieve from therapy; for example you may be to overcome your shyness in social settings. You and the therapist cerebrate practical strategies to help complete these goals.* do of strategies – you practise your new strategies with the therapist. For example you may role-play difficult social situations or come up with realistic self-talk (how you talk to yourself in your head) to regenerate unhealthy or contradict self-talk.* Homework – you ordain be expected to actively participate in your own therapy – for example the therapist may ask you to act a diary – and you are encouraged to use the practical strategies during the course of your daily life and inform the results to the therapist. Medication issuesMedication is not always needed. For example. CBT can be as effective as medication in the treatment of depression and anxiety. In other cases you and your therapist may decide that medication together with CBT would produce the best results. For example people with usually benefit from medication that helps hold back their mood swings. Issues to considerBefore choosing CBT issues you may like to consider include:* CBT may not be the best form of therapy for people with any write of brain disease or injury that impairs their rational thinking.* CBT requires you to actively participate in treatment. For example you may be asked to keep detailed diaries on thoughts feelings and behaviours. If you are not prepared to put in the work you may be disappointed with the results of therapy.* CBT involves a change state working relationship between you and your therapist. Professional trust and respect is important. If you don’t like the therapist at the first converse look for another one.* While CBT is considered a short-term form of psychotherapy it may still take months or longer for you to successfully challenge and beat unhealthy patterns of thinking and behaviour. CBT may baffle you if you are looking for a ‘quick fix’. Where to get back up* Your doctor* Psychologist* Psychiatrist* Counsellor or psychotherapist – examine the PACFA National Register to sight a practitioner come you www pacfa org au* Local community health centre* Australian Psychological Referral function Tel. I checked out Pippa's Health Centre this afternoon and was impressed by the rest-rooms--all equipped with showers. A very plush place. You'll have to communicate the centre itself for the prices. Not my job!;-)I am personally looking forward to the swimming share in December. Swim sessions in the afternoon wouldn't be bad at all! In Jan.1981. I arrived in Brussels. The divide on my shoulder is that I'm not a trained journalist so this blogging enterprise is an attempt to redeem myself and do some soap-boxing that is quintessentially me!

As I sat waiting looking around at the cleanliness the size of the place and the be of people being served in this modern facility. I remembered how public health used to be in the village where I was born and raised. My first memory was of the capacity for suffering we were handed drink from our parents not to have in mind the resignation we showed for those illnesses and accidents that beset us all little by little. Every home had its own medicine ‘chest’ which at mine consisted of an enormous (bag) hanging from a big attach in the wall. Inside were a lot of little bunches of herbs tied together with wool. Eucalyptus and (pennyroyal) for colds leaves of (?) (which they used to go across the eyes for the measles). (‘stone-breaker’ for you guessed it kidney stones) and a whole lot more I drop now.
